What is the Heart Failure program?
The Heart Failure program is a telephone-based support and coaching program for members who live with heart failure symptoms.
Heart failure is a condition that develops when the heart’s muscle becomes weakened. For many people, heart failure can be a debilitating condition where normal everyday tasks leave them breathless and exhausted. In most cases, heart failure cannot be cured, however, with careful management it can often be controlled.
This program is designed to help members who live with heart failure symptoms find ways to prevent and manage exacerbations, maintain quality of life and stay safe at home.
How does the Heart Failure program work?
The program, delivered over a 12-month period, aims to provide simple tips on how to recognise early warning signs and fluid retention symptoms, and helps you to understand common terms used by your doctor.
The program can help you

improve your knowledge of the condition, including risk factors, symptom recognition and management

ensure your treatment is in line with the national heart failure treatment guidelines

make small but important lifestyle changes to better manage your condition

increase your knowledge regarding your medication management

learn self-management skills and how to cope when short of breath
-
manage your quality of life, and minimise the need for hospitalisation and emergency department care

link into community-based services and supports that can help you to stay safe, well and supported at home.
This program is coordinated by our health partner Remedy Healthcare, an experienced provider of home and community based care.
Are you eligible for the Heart Failure program?
To access the Heart Failure program, you need to

hold an appropriate level of private hospital cover with Australian Unity
-
have held that level of cover for at least 12 months
-
have been diagnosed with heart failure or cardiomyopathy
