Medical authority
Complete this form if you are being referred for further tests or might require hospitalisation and if you’re within the first 12 months of joining Australian Unity or upgrading your cover
Accident report/Claim assessment
Complete this form if you’re receiving treatment that may be related to an accident, for example joint replacements, arthroscopies or hernia procedures

Don’t pay more than you need to
Make sure you receive all of the health insurance discounts and rebate you’re entitled to.
Australian government rebate on private health insurance
You might be eligible to receive a rebate from the government to help cover the cost of your health insurance
Transfer certificate request
If you’re switching from another health fund, make sure you pay the correct level of Lifetime Health Cover loading and that you don’t have to re-serve any waiting periods you’ve already completed
Direct debit request
Receive up to a 4% discount on your health insurance by arranging automatic payment from a nominated bank, building society or credit card account
