Call us on 1300 081 986 and we can discuss your health and wellbeing needs. We can develop a health care plan to suit you.

Call My Aged Care

Call My Aged Care on 1800 200 422 and let them know you would like to transfer your Home Care Package to Australian Unity and write down your new referral code.

Confirm and switch

Call us and confirm the date when your services will end with your current provider. We will then arrange a time to meet with you to finalise your personalised health care plan.

Arrange your start date

As soon as you have your agreed end date, call our Home Care team and we can arrange a start day for your services.
