Living with COPD
What is the Living with COPD program?
This program aims to help members living with Chronic Obstructive Pulmonary Disease (COPD) recognise warning signs, manage symptoms and maintain independence and quality of life.
How does it work?
This program, carried out by an experienced health coach over a 12-month period, is designed to help you self-manage and cope with your condition on a day-to-day basis.
Through the program, you can learn how to recognise the early warning signs and symptoms of a flare up, and the lifestyle changes that can help you stay well. You’ll also be given advice on when to seek appropriate early GP treatment and what you can do to minimise exacerbations.
In addition, the program can help you
- improve your knowledge of the condition, including risk factors
- ensure your treatment is in line with the evidence based guidelines of the Australian Lung Foundation
- make small but important lifestyle changes to better manage your condition
- increase medication management knowledge
- learn how to cope when short of breath
- manage your quality of life, and minimise the need for hospitalisation and emergency department care
- link into community-based services and supports that can help you to stay safe, well and supported at home
Are you eligible?
To access the Living with COPD program, you need to
- hold an appropriate level of private hospital cover with Australian Unity
- have held that level of cover for at least 12 months
- have been diagnosed with Chronic Obstructive Pulmonary Disease
