Terms and Conditions: Australian Unity employees Gift Card offer (7 October – 30 November 2024).

  1. By entering the “Gift Card” promotion (Promotion), you acknowledge that you have read these terms and conditions and that you agree to be bound by them. Information on how to enter and prizes forms part of these terms and conditions of entry.
  2. The promoter is Australian Unity Health Limited ABN 13 078 722 568, 271 Spring Street, Melbourne, VIC, 3000 (Promoter).
  3. The promotion runs from 12.01am (AEDT) 07/10/24 to 11:59pm (AEDT) 30/11/2024. The Promoter reserves the right to change the promotional end date at any time by publishing updated terms and conditions on its website.
  4. Gift card value is based on product coverage and scale:
    1. Hospital & Extras
      1. Singles $125
      2. Single Parent Family $250
      3. Couples $250
      4. Family $250
      5. Extended Family (Family Plus) $250
      6. Extended Single Parent Family (Family Plus) $250
    2. Extras
      1. Singles $30

  5. To be eligible for the promotion a person must:
    1. be an employee of Australian Unity; and
    2. Not have been a member of Australian Unity health insurance within the past 3 months.
  6. To be eligible to receive the gift card, entrant must take out an Australian Unity health insurance policy through the Australian Unity Call Centre or Corporate Partnerships team. Bookings can be made online at australianunity.com.au/healthperks or via phone on 03 8601 1416
  7. Subject to 8 below, a limit of one gift card per membership applies. There is no fee to enter. No late entries will be accepted.
  8. Gift Cards will only be issued after you have completed 60 continuous days of paid membership and your membership is paid up to date. After completing the eligibility period allow up to 30 days to receive your Gift Card. The gift card will not be issued to any policies that at any time during the first 60 days of membership have been or are:
    1. in arrears; or
    2. suspended.
  9. For the avoidance of doubt, this Promotion is not available:
    1. to the public generally or other corporate partnership members/subscribers; or
    2. through brokers, intermediaries, or comparators;
  10. The Promoter may require the winner to provide proof of:
    1. eligibility in accordance with paragraph 5 and 6;
    2. identity;
    3. age;
      Identification considered suitable for verification is at the Promoter’s discretion.
  11. The Promoter accepts no responsibility for any tax implications that may arise from winning the prize. Independent financial advice should be sought.
  12. To the extent permitted by law, if for any reason whatsoever this Promotion is not capable of being conducted as reasonably anticipated, including by reason of technical failure, fraud or any cause beyond the reasonable control of the Promoter, the Promoter reserves the right, in its sole discretion, to cancel, suspend, modify or terminate the Promotion .
  13. To the extent permitted by law, if for any reason whatsoever this Promotion is not capable of being conducted as reasonably anticipated, including by reason of technical failure, fraud or any cause beyond the reasonable control of the Promoter, the Promoter reserves the right, in its sole discretion, to cancel, suspend, modify or terminate the Promotion.
  14. Gift card issued by Cuscal Ltd ABN 95 087 822 455 AFSL 244116. Cuscal Ltd deals with any funds loaded onto the card in accordance with its arrangements with Corporate Prepaid Cards Pty Ltd ABN 25 092 828 772 AFSL 509642
  15. All gift cards are Universal Gift Cards Mastercard gift cards, issued by Cuscal Ltd ABN 95 087 822 455 AFSL/ Australian Credit License 244116. Cuscal Ltd, preloaded with up to AU$250 in cash only (as applicable). All Gift Cards will be delivered electronically or by post. Gift Cards are subject to the terms and conditions published at https://myaccount.universalgiftcard.com.au/terms.aspx.
  16. Gift cards are valid for 12 months as printed on the front of the gift card. Expired gift cards will not be replaced.
  17. Australian Unity will not be held responsible for any damaged or lost Gift Cards and no replacement will be provided. Australian Unity will replace a stolen gift card at its discretion in the event of fraud and / or misuse.
