Don’t let your diabetes manage you.
Our Diabetes Action program is telephone based and available to members with type 2 diabetes.
This program is run by experienced health professionals with the aim of increasing your confidence and knowledge to effectively manage your disease and reduce the risk of long-term complications.
Across the four to six month program of phone-based consultations, your health coach will help you review your nutritional knowledge and work with you on your activity and exercise regime, as well as helping you access the services, programs and health professionals in your community that can assist you to meet your health goals.
The program includes useful education and support on many risk factors that can affect living with diabetes such as diet, activity, medications, blood glucose management, hypo and hyperglycaemia, sick-day management and several other aspects of diabetes control.
This program is coordinated by our health partner Remedy Healthcare, an experienced provider of home and community based care.
