Gym Membership
What is it?
Gym membership benefits are to support the treatment of a specific health condition.
How does it work?
A referral is required from a recognised healthcare practitioner stating that the activity is being undertaken as part of a treatment program for a specific health condition. Recognised healthcare practitioners can be: a GP, medical specialist, nurse, chiropractor, osteopath, physiotherapist, occupational therapist, psychologist, or dietician. The recognised healthcare practitioner completing the referral cannot be associated (eg. work at the gym) with the business providing the gym service. The letter of referral must be dated within 3-months of the date of the first benefit claimed. Referrals are only valid for twelve-months from the date of referral.
Are you eligible?
To claim a benefit you need to:
- hold an eligible product that includes benefits for Gym Membership
- have held a product with that benefit for at least six months

How to claim
To claim, just send a copy of the receipt for your gym membership, containing your name, with a completed ‘Preventative Health Services – Claim Form’ to Australian Unity.
