Terms and Conditions: Get Wellplan Rewards credits when you purchase hospital and extras cover (1 October 2024 – 1 January 2025)

1. This offer only applies:
   1. to people who purchase Australian Unity hospital and extras cover at the same time (i.e. either a combined product or a hospital product and an extras product);
      1. through Australian Unity Partnerships (phone and online portals);
      2. between 1 October 2024 – 1 January 2025 where the policies purchased have a commencement date between 1 October –1 January 2025; and
      3. who are new members on new memberships (see condition 3 below); or
2. After complying with paragraph 1 above, and subject to paragraph 4 and 5 below, the policy holder may receive Wellplan Rewards credits (ie Singles $125, Couples and Single Parent Families $250, Families/Extended Families/Extended Single Parent Families $300) applied to their Wellplan Rewards account attached to their cover. Wellplan Rewards credit is limited to one per membership.
3. Offer only available to new members on new memberships, who have not previously held Australian Unity health insurance within 90 days of joining.
4. This promotion is not available:
   1. to members who take out hospital only cover or Overseas visitors Cover;
   2. to Australian unity staff (this includes Greengate21 staff;
   3. to dependants who come off their parent’s policy and take out hospital & extras cover or a combination cover within 90 days. However, they may be eligible for a separate offer if they join within 30 days, refer to the details here
5. The Wellplan Rewards credit will be applied to the member’s Wellplan Rewards account after the policy holder has held cover for a minimum period of 60 continuous days and the policy is paid up to date and the member has activated their Wellplan Rewards account within 90 days of their policy start date.
6. The Wellplan Rewards credit will not be applied on any policies that at any time during the first 60 days of membership have been or are:
   1. in arrears; or
   2. suspended
7. Wellplan Rewards credit requires members to activate their Wellplan Rewards account via Online Member Services (OMS). Members can activate their Wellplan Rewards account 31 days after purchasing health insurance. To activate members must login to Online Member Services, then click on REWARDS tab and then accept terms and conditions of Wellplan Rewards no later than 90 days after purchasing health insurance. Members who fail to activate their Wellplan Rewards account by day 91 will forfeit their Well plan Rewards credit.
8. Wellplan Rewards credits will expire after 6 months.
9. Member who leaves Australian Unity will forfeit any unused Wellplan Rewards Credit.
10. This offer is not available in conjunction with any other retail offer or promotion, except where those retail offers or promotions are clearly expressed or communicated by Australian Unity to constitute or form part of a single offer.
11. Those who join via a partnership relationship are eligible for the partnership discount and the Wellplan Rewards credit.

   1. purchase Australian Unity hospital and extras cover at the same time (i.e. either a combined product or a hospital product and an extras product) through Australian Unity Partnerships (phone and online portals) between 2 January – 30 September 2024 where the policies purchased have a commencement date between 2 January – 30 September 2024; and
   2. are new members on new memberships (see condition 3 below)
